Scheduling Tubal Ligation Reversal with Dr. Berger or Dr. Monteith
We are always available to answer your questions or to assist you in scheduling your tubal reversal. Our office is open Monday-Friday from 8AM to 5PM EST.

When you contact us, we will obtain the necessary information to confirm a surgery date. Most patients can be scheduled within four weeks from the date they contact our office.
If you have not already done so, please have your operative report sent here for review. If you do not currently have your operative notes, you can print and complete the Medical Records Release form and fax it to the medical records department of the hospital where your tubal ligation was performed. When sending your operative report, remember to complete and include the Operative Report Fax Cover form so we will be able to contact you after receiving your records.
Once you have a surgery date scheduled, print a copy of the Pre-operative Checklist to help you stay organized as you prepare for your reversal surgery and trip to Chapel Hill.
For maximum safety in the outpatient surgery setting, our policy requires patients to have a body mass index (BMI) of less than 35 at the time of surgery. BMI is based on height and weight and can be determined by using our BMI calculator. For your own safety, it is essential that you provide accurate information regarding your medical history when scheduling surgery and during your anesthesia consultation. Providing inaccurate information about weight, past surgery or medical conditions may result in your procedure being cancelled or rescheduled.
A responsible adult over the age of 18 must accompany you to the facility for your consultation and on the day of surgery. This person must remain with you in the facility during your consultation, your surgery, and at the hotel following your discharge from the surgery center. Because our waiting area is small, you must bring only one person - your responsible adult - with you on the day of surgery. For the safety, comfort, and privacy of all of our patients, children are not allowed to accompany parents to the facility.
