Tubal Reversal at Chapel Hill Tubal Reversal Center

At Chapel Hill Tubal Reversal Center, reversal of tubal ligation takes approximately one hour and no overnight hospital stay is needed. The advanced surgery techniques used by Dr. Berger and Dr. Monteith result in a low cost outpatient procedure that has produced the most tubal reversal babies born worldwide.

When you are ready to schedule your reversal surgery, payment of a $100 scheduling fee is required to be added to the surgery schedule. The scheduling fee payment is non-refundable. Payment can be made via PayPal at www.PayPal.com using the email: Payments@tubal-reversal.net as the email address where the payment is to be sent. Payment can also be submitted directly to us using bank-to-bank wire transfer, Visa, MasterCard, Discover card, cashier's check or money order. When we receive thescheduling fee, one of our Tubal Reversal Nurses will contact you to assist with scheduling your reversal surgery.

The Reduced Fee* for tubal reversal is $5900. To qualify for the Reduced Fee, payment is due in full at the time of scheduling.

If payment is not made in full when scheduling surgery, the Standard Fee of $6900 will be charged. With the Standard Fee, you can schedule your appointment and hold a date with a $250 down payment. The balance is due 3 weeks prior to the surgery date.

The surgery fee for reversing Essure or Adiana sterilization is $6900 and is due at the time the appointment is scheduled. We offer a reduced fee for Essure 'coil removal only' and details are available by calling us at (919) 968-4656.

The surgical fee payment can be made by bank-to-bank wire transfer, Visa, MasterCard, Discover card, or sending a cashier's check or money order by over-night delivery. We do not accept payment for the surgical fee through PayPal. Payments via credit and debit cards will be subject to an additional $100 processing fee.

The Tubal Reversal Surgery Fee is all inclusive and covers:

• Preoperative record review and consultation
• Surgeon's fee
• Anesthesia fees
• Surgical supplies
• Operating facility fees
• Postoperative pain medication
• Follow-up care

Insurance and Tubal Reversal

Some insurance companies may cover part of the cost of tubal ligation reversal. If your insurance carrier will cover part or all of the cost of your surgery, please be sure they send the reimbursement check to you directly. The insurance code (ICD-9) for bilateral tubal occlusion is 628.2. The procedure code (CPT) for tubotubal anastomosis is 58750. We are a fee for service practice and do not accept payments from insurance carriers or file insurance claims.

Any pre-certification or pre-authorization that may be required by insurance policies are the responsibility of the patient. Since our standard and discount fees are all-inclusive, we do not provide a cost breakdown to insurance companies.

Pre-payment Account Option

For patients interested in a long-term payment arrangement, our office offers a pre-payment account option.

Payments for the pre-payment account should be made by cashier's checks or money orders made payable to Chapel Hill Tubal Reversal Center. We do not accept PayPal payments for pre-payment accounts.

There is a non-refundable processing fee of $250 to open a pre-payment account that will go toward the surgery fee. If the account is closed without the patient having surgery, our office will retain the $250 processing fee.

To utilize the pre-payment option, complete the Pre-Payment Form and mail it with your cashier's check or money order made payable to Chapel Hill Tubal Reversal Center to:

Once you have opened a pre-payment account, you may send additional payments of any amount greater than $10 as you are able. Each time you send a payment, complete the Pre-Payment Form, print it, and mail it with your payment. You may receive the reduced fee while using the pre-payment account option, but the balance must be paid in full prior to scheduling a surgery date. For the standard fee, you do not have to wait until the balance is paid to schedule surgery, but the balance will be due in full 3 weeks prior to the scheduled surgery date. Accounts are considered inactive if no payment is made over a period of 12 months. In this case, patients are no longer locked into the fees that were in place at the time they began the account and current fees will apply. Cancellations and Rescheduling

Cancellation of a procedure within 8 to 21 days of the surgery date will result in a 25% cancellation fee being retained by CHTRC. Patients may reschedule their surgery date one time, outside of 3 weeks of the date, at no charge. There will be a rescheduling fee of $1,000 for each additional date change and for any date change that occurs within 14-21 days of the surgery date. Rescheduling or canceling of surgery within 8-13 days of the surgery date will require an additional fee of 25% to reschedule. Rescheduling or canceling within 7 days of the date of surgery, regardless of the reason, will result in forfeiture of 100% of the surgical fee. Refunds on cancellations can take up to 30 days to process.

* The reduced fee is a one time offer. If you cancel your procedure for any reason and reschedule at a later date, the standard fee will apply. If you start a Pre-payment plan with us you are still eligible for the reduced fee.
