$5900 reduced fee

The all-inclusive cost or fee for tubal reversal performed by Dr. Berger is $5900 when payment is made in full within 24 hours after scheduling your surgery. This is a $1000 reduction from the standard fee. Some insurance companies may cover part of the cost of tubal ligation reversal.

The standard fee does not require full payment at the time of scheduling but must be paid at least three weeks prior to the date of surgery.

Payment can be made by Visa, Master Card, Discover Card, bank wire transfer, or by sending a cashier's check or money order via overnight delivery. Personal checks or Western Union transfers are not accepted. Patients who submit payment for the entire surgery fee by wire transfer, cashier's check or money order will receive an additional $100 reduction in the total surgery fee.

What is included

Both the standard and reduced cost are all-inclusive and cover:

• Preoperative record review and consultation
• Dr. Berger's surgical fee
• Anesthesiologist's and nurse anesthetist's fees
• Surgical supplies
• Operating facility fees
• Postoperative pain medication and antibiotics
• Follow-up care.

The reduced fee of $5900 is a one time offer. If you cancel your procedure for any reason and reschedule at a later date, the standard fee of $6900 will apply. If you start a Pre-payment plan with us you are still eligible for the reduced fee. When you have $5900 in your account you may then schedule your surgery date at the lower price.

There is a $250 non-refundable charge due at the time of scheduling. This charge will be applied to the total fee.

Insurance and tubal reversal

Some insurance companies may cover part of the cost of tubal ligation reversal. We are a fee for service practice and do not accept payments from insurance carriers. If your insurance carrier will cover part or all of the cost of your surgery, please be sure they send the reimbursement check to you directly. The insurance code (ICD-9) for bilateral tubal occlusion is 628.2. The procedure code (CPT) for tubotubal anastomosis is 58750. Any pre-certification or pre-authorization that may be required by insurance policies are the responsibility of the patient. Since our standard and discount fees are all-inclusive, we do not provide a cost breakdown to insurance companies.

Pre-payment option

This option permits you to pre-pay the surgical fee in installments without interest prior to the surgery date. To utilize this option, send a non-refundable $250 payment to:

An account will be opened in your name, and you can send additional payments of any amount as you are able. If you would like to start a pre-payment account or would like to make a payment toward your existing account, complete the Pre-Payment Form, print it, and mail it with your payment each time you make a payment toward your account. You are welcome to schedule your procedure at any time. You do not have to wait until the balance is paid to schedule surgery, however, the full fee is due at least three weeks prior to the surgery date. If you wait to schedule until you have $5,900 in your account, then you may schedule a date at the reduced fee. Cancellations and Rescheduling

There is a 25% cancellation charge if you cancel within 8 - 21 days of the date of your tubal reversal procedure and a $1,000 rescheduling fee within this time frame. Rescheduling or canceling of your surgery within seven days of the surgery date, regardless of the reason, will result in forfeiture of 100% of the surgical fee. You can reschedule one time outside of three weeks without charge. However, subsequent rescheduling will result in a $1,000 fee.
