Pregnancy report forms

Use the Pregnancy Report Form when you first realize you are pregnant. We want to be informed about every pregnancy and the information you provide is vital to our pregnancy statistics. We want to be informed about every pregnancy of every patient who has had surgery at our center.

Tubal reversal update form

Use the Tubal Reversal Update Form to let us know how your pregnancy is going. We want to know about the pregnancy progress of each of our patients and this form is the best way to keep us updated.

Not only do we want to be informed about every pregnancy, but we also want to be updated on the status of your pregnancy. We take pride in our work and we enjoy seeing our tubal reversal babies grow up. Update us on your reversal baby at anytime by submitting an update form.

Keep us updated about significant event in your baby’s life and feel free to attach ultrasound, baby shower, birthday, and graduation pictures to share your experience with us. Your feedback validates the work we do and also provides hope for others who may be considering corrective tubal surgery.

Tubal reversal birth form

Use the Tubal Reversal Birth Report Form to let us know when your baby is born. Once we receive notice of your baby’s birth we will send you a ‘Monteith Miracle’ t-shirt for your newborn baby. We love to see pictures of our reversal babies so make sure you attach one with your report!

Tubal ligation symptoms report

Use the Tubal Ligation Symptom Update Form to update us on your symptoms after surgery at our center. Many women report abnormal symptoms after tubal ligation. If your tubal reversal surgery was to evaluate abnormal symptoms after your tubes were tied then complete this form to let us know how you have been doing since having surgery at our center.

Many women have been told ‘you are getting older’ or ‘its all in your head’ so your feedback about your personal experience is both insightful and helpful to others who may be experiencing similar symptoms.

I am 45 and i had 4 kids already, had my tubes tied 15 yrs ago, i recently got married and my husband has no kids and would love to have one is it possible for me to get my tube un tied

    It is harder…but not impossible… to become pregnant with increasing age.

    Your chances of pregnancy at age 45 are less than 8%… but don’t wait too long if you want tubal reversal. Each year you wait you lose 12 chances and each year the chances become less.

    If you are having periods then each month you have a chance to become pregnant. Each year you have 12 chances. The older you become and the longer you wait…then each year you lose 12 yearly chances and each monthly chance becomes less every year you wait.

    We do have many patients who become pregnant with tubal reversal after age 40. Most of the women who become pregnant after reversal are between the age of 40 and 43.

    We charge $6,900 total plus $150 to schedule and we are located in Raleigh, North Carolina.

    Patients pay in full and out-of-pocket at the time of scheduling. All you have to pay for in addition to the surgical fee is the cost of travel, hotel, and food.

    The alternative treatment is in-vitro fertilization (IVF). Most IVF doctors will recommend you use donor eggs after the age of 42. A single cycle of IVF averages $14,000 and average success is 38% for each try. After age 40 IVF success decreases quickly.

    If you would like to get started then the best first step is to send us your tubal ligation records for a free review. Your tubal ligation records can be obtained from the Medical Records Department in the hospital where your tubal ligation was performed. Occasionally your doctors office may have the records. Sometimes patients may not be able to obtain their tubal ligation records. We can still proceed with surgery without records because we can reverse most tubal ligation procedures.

Is it possible with you guys to have a tubal reversal when I had 2 6cm×.06cm with fimbriated ends and 1.3cm×.05cm without fimbriated ends removed? I told my doctor after being heavily forced into a tubal at 25 that I want to have a reversal at 30 she assured me that with this tubal it was possible I went and seen my ob last week to start making preparations and was told that with this tubal I had no chance of reversal he also commented heavily on how many kids I already had so I looked into my pathology report because I felt he was dishonest and that was my findings is it still possible to have a reversal done?
