PREPAYMENT ACCOUNT
Creating a Reversal Prepayment Account allows you to contribute money gradually over time towards their reversal surgery. When you have the full amount you can schedule your reversal surgery. The initial contribution must be $500 or more and the account must be fully funded within three (3) years of opening.
Discounted tubal reversal surgery
By opening a Reversal Prepayment Account (RPA) we realize you are making a significant commitment to our practice.
To show our appreciation for this commitment, we are going to provide our RPA holders exclusive access to discounted surgery rates when these discounted rates become available. We will also lock in the surgery fees for RPA account holders for the fee quoted at the time the account was started. This means, if we increase our surgical fee at any time in the next three (3) years, RPA holders will be guaranteed the surgery fee quoted at the time they started their account.

- You can use the RPA as a savings plan to gradually save for your reversal surgery.

- You can have your surgery fee locked in and protected from future fee increases.
- You will be notified about discounted tubal surgery rates when these rates are available. Discounted rates will only be available to RPA holders.
Starting an RPA: 5 Simple Steps!
Starting an RPA is a great way to save for tubal surgery and you may be able to take advantage of a discounted rate when offered.

Step 1. Complete RPA prescheduling questionnaire
The key to starting an RPA is to complete our prescheduling process. This allows you to make sure you will be the best candidate for reversal surgery before you start saving. It also allows us to evaluate you for any health conditions which could prevent you from having reversal surgery and identify any conditions which could be improved or optimized before your reversal surgery.
By completing the prescreening process you will be in a better position to take any discounted surgery rates should they become available on short notice.

Step 2. Pay surgery scheduling fee
After you have submitted the Prescheduling Questionnaire you will need to pay the $175 scheduling fee. You have 90 days to start prescheduling after paying the scheduling fee.
If 90 days elapses and you have not begun prescheduling then you will need to repay the surgery scheduling fee.
You cannot use CareCredit to pay the scheduling fee.

As an RPA holder, you will not have to pay the scheduling fee a second time when you are ready to schedule your surgery.
Step 3. Complete prescheduling phone health interview with a nurse
This is important because it will allow us to screen for common medical problems before you open your RPA. This will help identify unanticipated problems that can often arise when patients are trying to scheduling surgery (over BMI limit, unexpected anemia, pre-operative clearance for complicated medical conditions, and undiagnosed health issues which could be improved while you are saving for surgery).
At the end of the interview the nurse will ask you to obtain a complete blood count (CBC).
We can order this test for you or you can send us a recent (within 3 years) CBC result from your medical records. If any significant medical problems are identified, we may require additional tests and our team can make the appropriate recommendations to prepare you for surgery. We can also provide you with recommendations on the best way to prepare for a safe surgery and a healthy pregnancy while you are making contributions towards your RPA.
You or your health insurance will be responsible for the cost of any required lab testing. Often your health insurance will pay for required labs with a doctor’s order.
When you are ready for surgery, you must repeat the scheduling interview and obtain a second CBC test within 90 days of your scheduled surgery.
Step 4. Return the RPA contract and initial contribution
After your scheduling interview has been completed and our office has received all required lab tests, x-rays, and medical clearances then our office will send you an RPA contract by email.
Return the completed and signed RPA contract within 30 days of completing your Prescheduling interview with any amount greater than $500 to secure your locked in surgery fee and to start your RPA account.
You will not be officially considered an RPA holder until you have paid the scheduling fee, completed the prescheduling interview, and returned the initial RPA contract and deposit. Only then will you be notified of discounted rates when available.
Step 5. Contribute to your RPA
Contribute to your RPA when you are able. You must send a minimum of $100 with each contribution. We recommend you consider an average of $150 per month to fully fund the average surgery fee in full by the end of 36 months (3 years).
Important points about the RPA
- You can schedule your surgery at any time but you must schedule your surgery within three (3) years of starting the account.
- Our office will send monthly emails to RPA members notifying them of reduced rates if these rates are available. We will provide as much advanced notice as possible.
- Since you have completed the prescheduling interview then most problems should have been identified and you can save towards your surgery with the confidence you are going to be the best candidate possible for both surgery and pregnancy.
- The scheduling fee to start a Prescheduling interview for an RPA must be paid using a credit card; however, we are unable to accept contributions towards the RPA using credit card.
- Contributions to the RPA should be made by cashier’s checks or money orders made payable to Charles W. Monteith, MD PA.
Making future contributions to your prepayment account
To send future contributions, use the Prepayment Form, on our website and mail it with your cashier’s check or money order.
The entire surgery fee must be paid in full before you can schedule a surgery date.
We do not accept credit card payments for RPA contributions.
