Surgery dates cannot be confirmed until all signed documents are returned and full payment is received.

Important surgery policies

Step 1. Complete Surgery Prescheduling Questionnaire
Completing this questionnaire helps the scheduling nurse quickly complete your phone interview and makes the scheduling process more efficient.

After completing the questionnaire you will be able to automatically email the completed questionnaire directly to the scheduling nurse.
Step 2. Pay scheduling fee
After completing the Surgery Pre-scheduling questionnaire, you will need to pay the $175 non-refundable surgery scheduling fee.
You cannot use CareCredit to pay the scheduling fee.

Step 3. Scheduling phone call
After completing the Surgery Prescheduling questionnaire and paying your scheduling fee, a scheduling nurse will call you within 24 hours to conduct a health interview and schedule your surgery.
If you prefer not to wait for our scheduling nurse to call then you are welcome to call during East coast business hours to schedule your surgery. Before calling, you should locate a local laboratory (such as LabCorp) and obtain the phone and fax number. You should have this information available to provide the nurse during your scheduling interview. It is important to provide a thorough, detailed health history when you speak to the nurse. Providing inaccurate health history or withholding important health information will result in cancellation or rescheduling of your appointment and forfeiture of surgery fees.
During your scheduling phone call, the scheduling nurse will obtain a comprehensive health history and also conditionally confirm your date of surgery. The phone health interview should take less than 20 minutes to complete.
Screening laparoscopy
If you are scheduling tubal ligation reversal surgery, our scheduling nurse will also discuss the optional screening laparoscopy with you during your interview.
Preoperative lab testing
Tubal ligation reversal and Essure surgery patients are required to have a complete blood count (CBC) test to check for anemia (low iron) performed within 90 days of their surgery date. Depending on your medical history, additional tests may be required.
If your surgery date is within the next 90 days, you should have the required pre-operative lab testing performed within 48 hours. The scheduling nurse will fax an order to any lab of your choosing using the information you provided when you called to schedule your appointment.
Step 4. Return signed confirmation letters
The Scheduling Nurse will email you a Confirmation Letter, Patient Responsibility Statement, and Registration Packet soon after your phone health interview is completed.
Read through the documents you received by email. Sign and date each of the signature pages and return these by scanning and emailing or by fax within 24 hours.
Step 5. Pay surgery fee and complete lab tests
Submit payment of your full surgery fee within 24 hours via MasterCard, VISA, or Discover or through a bank-to-bank wire transfer. You may also send a cashier’s check or money order by overnight mail.
- Most patients will find bank-to-bank wire the most convenient and most cost effective way to send payment. Call our office for wire instructions.
- Payments submitted by credit card will be subject to an additional processing fee.
Your surgery is confirmed when…
Your documents have been signed and returned and your full payment has been received by our office can you consider your surgery date confirmed and scheduled.
Do not make hotel or travel reservations until our office has confirmed receipt of all signed documents and full payment for surgery.
