REVERSAL PREPAYMENT ACCOUNT
Creating a Reversal Prepayment Account (RPA) allows you to contribute money gradually over time towards your reversal surgery. When you have the full amount you can schedule your reversal surgery.
- Your initial contribution must be $500 or more and the account must be fully funded within three (3) years of opening.
- If your surgery has not been scheduled within three (3) years of starting your RPA then your RPA account will become inactive and will be closed. You will be refunded your monies less a $500 RPA administration fee.
Locked in fee and discounted tubal reversal surgery when available
By opening a Reversal Prepayment Account we realize you are making a significant commitment to our practice.
To show our appreciation for this commitment, we are going to allow our RPA holders to lock in their fee when they start their RPA and they will have exclusive access to discounted surgery rates when these discounted rates become available.
- Prescreening. When you first start an RPA you will go through the first half of the scheduling process. This allows us to prescreen RPA members to make sure they are they best candidates for reversal surgery.
- Locked in fee. You will be protected from surgery fee increases during the three years your RPA is active. Your surgery fee cannot be increased when you schedule with an active RPA.
- Surgery discounts. When you start an RPA we make sure you are the best candidate for surgery. As a result of being prescreened, RPA members are in a good position to take last minute surgery spots when they become available. RPA members will be offered discounted rates when these last minute surgery spots become available.

Starting an RPA: 3 Simple Steps!
Starting an RPA is a great way to save for tubal surgery and you may be able to take advantage of a discounted rate when offered.

Step 1. Complete RPA prescheduling questionnaire and pay $175 scheduling fee
The first important step to starting an RPA is to complete our prescheduling process.
This allows you to make sure you will be the best candidate for reversal before you start saving. It also allows us to evaluate you for any health conditions which could prevent you from having reversal surgery and identify any conditions which could be improved or optimized before your surgery.
By completing the prescreening process you will be in a better position to take discounted surgery rates should they become available on short notice.

RPA Scheduling Fee
When you submit the RPA Prescheduling Questionnaire you will need to pay the scheduling fee. You have 90 days to start your RPA prescheduling after submitting the questionnaire and paying the scheduling fee.
If 90 days elapse and you have not completed your RPA prescheduling then you will need to repay the surgery scheduling fee.
As an RPA member, you will not have to pay the scheduling fee a second time when you are ready to schedule your surgery.
Once you submit the RPA questionnaire and pay the scheduling fee you have ninety days to complete the phone medical interview with our nurses.
If you have not completed phone interview within ninety (90) days then you will have to restart the entire process and pay the scheduling fee a second time.
Step 2. Complete prescheduling phone health interview with a nurse
This is important because it will allow us to screen for common medical problems before you open your RPA. This will help identify unanticipated problems that often arise when patients are trying to scheduling surgery (being over BMI limit, having unexpected anemia (low iron), needing a pre-operative clearance for a high risk medical condition, and having an undiagnosed health issue which could be improved while you are saving for surgery).
At the end of the interview the nurse will ask you to obtain a complete blood count (CBC).
We can order a CBC test for you or you can send us a recent (within 3 years) CBC result from your medical records. If any significant medical problems are identified, we may require additional tests and our team can make the appropriate recommendations to prepare you for surgery. We can also provide you with recommendations on the best way to prepare for a safe surgery and a healthy pregnancy while you are making contributions towards your RPA.
You or your health insurance will be responsible for the cost of any required lab testing. Often your health insurance will pay for required labs with a doctor’s order.
When you are ready for surgery, you must repeat the scheduling interview and obtain a second CBC test within 90 days of your scheduled surgery. You will not have to pay the scheduling fee a second time.
Step 3. Return signed RPA contract and initial contribution
After your scheduling interview has been completed and we have received all required lab tests, x-rays, and medical clearances then our office will email you an RPA contract.
Return the electronically signed RPA contract within 30 days of completing your RPA prescheduling interview and send your initial contribution of at least $500 or more to secure your locked in surgery fee and to start your RPA account.
Your RPA account will not be active until you have completed the RPA questionnaire, completed the prescheduling interview, and returned the initial RPA contract and deposit. Only then will your RPA become active and you will be notified of discounted rates when available.
After your phone interview has been completed, you have thirty (30) days to electronically return your signed RPA contract and your minimum contribution of $500.
If you have not returned the signed RPA contract with your financial contribution within 30 days of your completed phone interview then you will have to restart the entire process and pay the scheduling fee a second time.
Contribute to your RPA
Contribute to your RPA when you are able. You must send a minimum of $100 with each contribution. We recommend you consider an average of $200 per month to fully fund the average surgery fee in full by the end of 36 months (3 years).
Important points about the RPA
- You can schedule your surgery at any time but you must schedule your surgery within three (3) years of starting the account.
- Our office will send monthly emails to RPA members notifying them of reduced rates when and if these rates are available. We will provide as much advanced notice as possible.
- Since you have completed the prescheduling interview then most problems should have been identified and you can save towards your surgery with the confidence you are going to be the best candidate possible for both surgery and pregnancy.
- The scheduling fee to start a Prescheduling interview for an RPA must be paid using a credit card; however, we are unable to accept contributions towards the RPA using credit cards, CareCredit, or HSA/FSA cards.
- Contributions to the RPA should be made by cashier’s checks or money orders made payable to A Personal Choice PA
We are unable to accept credit cards, CareCredit, or FSA/HSA cards for contributions towards your RPA; however, when you are ready to schedule surgery you can use any of these methods to complete full payment for your surgery.
Making future contributions to your prepayment account
You can send in any contribution greater than $100. If you would like to wire money instead then please call the office for wire instructions.
