We recently changed our name from Chapel Hill Tubal Reversal Center to A Personal Choice Tubal Reversal. We also changed our website.

We still have our pregnancy report forms but we have made them even better and put them in a new location!

New Tubal Reversal Reports: Where are they?

The tubal reversal report section has been moved and is located at the very top of on all our website pages. The reversal reports can be found under the section “Patient Reporting”

New report: Pregnancy Update

We added a new report: Reversal Pregnancy Update

This report allows patients an easier way to update us on the outcome of their reported pregnancy. This information is important because it allows us to know what is happening with our patients. This information has also been important to determining the chances of pregnancy after tubal reversal surgery.

Tubal Ligation Symptoms Report

It was clear we had many who were coming to our center for treatment of abnormal symptoms after tubal ligation. It also became clear we did not have a good way to track of these patients to see how they were doing after tubal repair surgery.

To help us better follow the symptoms of our tubal reversal patients, we created a
Tubal Ligation Symptoms Update Form.

Tubal Reversal Birth Report

Although many doctors will support vasectomy reversal, many of these same doctors believe tubal reversal does not work and they advise their patients not to waste their time with reversal surgery.

Our birth reports prove these people wrong because these are real Tubal Reversal Testimonials, from real reports, submitted by real people who have real babies after reversing their tubes. What could be better proof tubal reversal works than by having women submit their stories and photos of their babies after tubal ligation reversal?

We are mindful fertility and pregnancy are private matters and we respect the wishes of all of our patients. This is why we ask if we can place their comments and baby photos on our website. If a patient request we do not share their information then we respect their wishes.

To better safe guard patient privacy we have added a signature section asking patients for a digital signature before we put their comments or baby pictures on our website.
