2. Send your tubal ligation report for a free evaluation
You can send your records about your tubal ligation surgery or tubal blockage to us for a free evaluation. We will review them and confirm if you are a candidate for tubal surgery.

3. Pay scheduling fee and schedule your surgery
After paying your $125 scheduling fee it only takes a 20-minute phone call to have a scheduling nurse interview you and schedule your surgery date. This fee can conveniently be paid with PayPal.

4. Complete the registration forms
After your scheduling interview is completed, you will be sent a confirmation email which will contain: a Patient Responsibility Statement, What To Expect brochure and Registration Packet. This information will educate and prepare you for surgery at our center. The signature pages of these documents should be signed and returned to our office.
5. Pay surgery fee
The surgery fee must be paid in full within 24 hours of scheduling. Only after payment is received will your surgery date be confirmed. The surgery fee can be paid with cashiers check, money order, credit card, or bank to bank wire transfer.
6. Obtain the pre-operative lab testing
We have greatly simplified the required testing for tubal reversal. We only require a complete blood count. We can order this lab test for you during your scheduling interview or your local doctor can order this test for you. This testing must be done within 90 days of surgery and the test results need to be faxed to our office.
If you have a complex medical history additional lab testing may be required and this will be discussed during your scheduling interview.
7. Make hotel and travel arrangements
Our office is in Raleigh, North Carolina, is conveniently located, and easily accessible by most airlines and Amtrak train. We are a short drive by car from all major cities on the East Coast and central US.

The hotels are located in the trendy North Hills Mall in mid-town Raleigh, are close to our specialty center. The hotels are within walking distance to restaurants, shopping, and entertainment.
These hotels offer patients of Dr. Monteith special room rates. In order to receive this special pricing, we recommend you make your hotel reservations with the hotel reservation email link which will be emailed to you after you schedule your surgery date.
